Social Security new client questionnaire - Social Security requires you to provide certain information about your past work, your medical treatment and your disabling condition. Our intake form, which is in Adobe pdf format, will help you organize this information. You can download our questionnaire in MS Word Format here. Bring your completed form to us, or, if it is more convenient, fax or mail it to us. Our fax number is 770-393-0240.
Appointment of Representative form - in order for our law firm to represent you, you will need to complete an Appointment of Representative form. You can download this form in Adobe pdf format.
Attorney-client fee contract - we handle most Social Security cases under contingency fee. This means that you pay no attorney’s fees unless we win a lump sum payment for you. You can download and review our fee contact here. Please note, however, that no attorney-client relationship is created until both you and one of our lawyers agree to the representation and both sign the fee contract and the Appointment of Representative form. When we accept a case, we will mail you a letter confirming our representation and copies of the fee agreement and Appointment of Representative forms signed by both you and one of our lawyers. Since time deadlines are short for filing appeals and other documents with the Social Security Administration, you should not assume that we will take any action on your behalf unless and until you receive written confirmation from us that we are representing you.
